The small monument consists of a granite stele approximately 70 cm high and a square tiled base measuring 130 x 130 cm. The stele is polished on the front side, but the edges and back are unworked and irregular in shape. A rectangular memorial plaque of bronze imitation is mounted on the front of the stele. On it is a depiction of the disappeared synagogue with the Magen David symbol in the window of the depicted temple and small silhouettes of human figures in the foreground. These motifs, as well as dedicatory inscriptions in Czech and English, are raised from the surface of the plaque.

The monument to the disappeared synagogue and Jewish community was unveiled in Přeštice on 15 September 2024. It is located on the corner of Husova and Červenkova Streets, where the former synagogue stood until 1974. It was built in 1913 for the needs of the local Jewish community, whose history dates back to the 16th century. The synagogue served its purpose until the Nazi occupation of Přeštice. The local Jewish community was dissolved in 1939. Most of the Jewish residents were deported to Nazi concentration and extermination camps in January 1942. Whole families were murdered. Of the several dozen people, only three survived the Nazi persecution. After World War II, the synagogue building was sold to the Evangelical Church of Czech Brethren, which used it as a church until the 1970s. In 1974, the building and the neighbouring house had to give way to a new housing development, which completely changed the urban character of the place. Nothing remains of the former building.
According to Michal Tejček, the expert consultant of the initiative to build this monument: "The idea for the creation of the monument originated long ago in the House of History of Přešticko and the Association for the Preservation of Historical Monuments of Přešticko. It was realized only thanks to the initiative of the Jewish congregations from Sharon, Massachusetts, USA, which together with other congregations and the Westminster Synagogue in London financed the creation of the memorial plaque. The monument was funded by the town of Přeštice and the aforementioned Jewish congregations from USA and UK." The graphic design of the memorial plaque was created by artist Jonatan Müller. The creation of the stele and the installation of the plaque was carried out by the stonemason Josef Měsíček. The memorial plaque was produced by the HVH artistic foundry in Hostinné. The unveiling ceremony, which took place under the auspices of Mayor Tomáš Chmelík, was attended by foreign donors, representatives of the Jewish community and Christian churches. The ceremony was part of a commemorative event during which 24 Stolpersteins were also placed in the town.
